Who is the candidate for sex change MTF surgery?

A suitable candidate for sex change MTF surgery is a person who has been diagnosed with at least one of the following disorders:

gender identity disorder
gender dysphoria
associated conditions

What are the requirements to have sex change MTF surgery?

As our doctors follow Transgender sex change guidelines from "The Harry Benjamin International Gender Dysphoria Association's Standards of Care for Gender Identity Disorders" a patient must meet the following requirements before having the surgery:

1.) Legal age of maturity age in the patient's nation.

2.) Psychology certificate stating that you are a suitable candidate for sex reassignment surgery by a psychiatrist or clinical psychologist who is a professionally qualified specialist in the field of gender dysphoria.

3.) Hormone therapy certificate stating that you have been on hormone for at least 1 year.

Where can I take hormone treatment?

A patient should see an obstetrician-gynecologist to ask for a hormone prescription. Side effects can be bad if there is over dosage. It is not advisable to take hormone on your own since you should be monitored for side effects while taking it.

Where can I get the psychology certificate?

A patient of sex change surgery should see a psychiatrist or clinical psychologist who is a professionally qualified specialist in the field of gender dysphoria.

What are the effects of estrogen hormone?

Genetic males treated with estrogen may expect the following results: breast growth, some fat redistribution to approximate a female body, decreased upper body strength, softening of skin, some decrease of body hair, slowing or stopping of scalp hair loss, decreased fertility and testicular size, less frequent and less firm erections. These changes are reversible, except for breast growth which may not regress after hormones are discontinued.

How is the sexual life after the surgery?

After sex change surgery, a patient can have normal sexual sensation. Sexual arousal is not only a physical sensation but it is also very much a psychological experience. It means that for you to have a fulfilling sexual experience, you have to be ready in both aspects. During removal of the penis and testicles, the skin, together with its nerve supply, are retained and used to line the new vagina. Also, a new clitoris will be formed from part of your glans penis (head of the penis) also with its nerve supply intact. Therefore, with this in mind, sexual sensation is still guaranteed.

How long should I stay in Thailand for sex change surgery?

In order for a patient to have sex change surgery, the patient should stay inThailand at least 2-3 weeks to complete a whole process and allow the proper healing. On an arrival day in Thailand, a patient should stay in a hotel to relax from a long flight and have a surgery on the following day. Gender reassignment surgery from male to female will require a patient to stay in the hospital for Approximately 14 nights.

How is surgery performed?

The sex change surgery is safe and effective. It takes approximately 4-8 hours. The procedure begins with the surgeon creating the vaginal cavity between rectum and the urethra by cutting some of the pelvic floor muscles to insert the neo-vagina. The surgeon then makes an incision to open the penis. The urethra is shortened and re-routed to emerge lower, just above the new vaginal opening. After the vaginal cavity is created, the surgeon cuts the scrotal skin to removes the testicles. The scrotal skin including the surrounding tissue is then used to construct the external genitalia such as labia minora and labia majora to create a neo-vagina similar to the biological female genitalia.

The surgeon uses the “Penile Inversion Vaginoplasty” technique which turns the penile skin inside out to line the vaginal wall. This technique limits the depth of the vagina depending on the amount of the penile skin available. In patients with short or small penis or who have had circumcision, the surgeon uses the scrotal skin graft in conjunction with the penile skin to increase the vaginal depth, usually 6-7 inches. The surgeon then constructs the clitoris by retaining a small section of the glans penis with its blood supply and nerves intact, since the nerves of the glans penis is analogous to the nerves of the clitoris in female. This is then positioned above the urethral meatus. At the same time the labia minora is created using part of glans penis and prepuce skin.

What is the post-surgical care?

After sex change surgery, the patient will be under the surgeon’s close supervision and he will inform you of everything you need to know. The following post-operative care guidelines should be observed:

The first 2-3 days the patient will be on a clear liquid diet and must avoid fibers and milky beverages that can induce defecation to avoid infection.
At bedtime, use the wrap to lie between legs while sleeping to prevent the wound being pressed.
Use a vaginal douche or sterile solution to clean the wound to prevent infection.
The patient will be able to walk after 5-7 days
Use a small size dilator and gradually increase in width and size, as you feel fit. You should dilate the neo-vagina 3-4 times a day. Dilation is needed everyday for 2-3 months then once a day after one year.
2 weeks after surgery return for a follow-up check and removal of stitches.

You should not engage in sexual intercourse until at least 2 months after the surgery.

What are the possible risks and complications?

The possible main risks are swelling, bruising, bleeding, infection, scarring and numbness, or change in sensation. Following the surgeon’s advice will reduce risks and complications.

Just open the yellow pages, magazines, brochures or view news paper advertisements and you’ll find plastic surgeons promoting all kinds of society memberships and board certifications but don’t be confused between boards and societies. Societies are merely professional associations. Board certification is the minimum criterion you should use in selecting a plastic surgeon. So, it is very important that you learn about the doctor’s education, training, certification, and experience in order to guarantee a successful outcome and to make the best choice.

Beware of lots of Letters after the Doctor’s Name:
Many fraudulent doctors’ have quite a few letters after their names. In fact, multiple letters may be the telltale sign of an inadequately trained doctor trying to impress the public. Definitely, your doctor should have the letters M.D (Medical Doctor) or D.O (Doctor of Osteopathy) after his name. He might even have one or more master’s degrees or might also have PhD but these degrees are irrelevant to plastic surgery. Patients are advised to select a board certified plastic surgeon.

Guide to choose a Plastic Surgeon:

Get Referrals: Begin your search for a plastic surgeon by asking friends, relatives, family doctor, colleagues that are willing to meet with you and share their results of the surgery. Hospital staff such as nurses, surgeons and anesthesiologist is also excellent sources of referrals. You may also do research on the internet, magazines, or in the yellow pages. Chances are a few names will stand out from the others. It will help give you the best possible recommendations.

Check the qualifications of the surgeon: One of the most important qualifications you should look for in a plastic surgeon is that they are board certified but you must need to know what board has certified your plastic surgeon and be sure that the surgeon has graduated from an accredited medical school.

Meet the surgeon: Look at pictures of past procedures done by the surgeon. Choose an ethical and confident surgeon and friendly staff members for a long lasting relationship and you should expect good communication, understanding and comfort from the surgeon.

Male to Female Sexual Reassignment Surgery

Male to female surgical procedure:
Sex change surgery for male to female involves the removal of the penile muscle and testicles reshaping it to appear more female thereby constructing a vagina. An incision is made into the scrotum, the flap of skin is pulled back and the testis is removed. The skin is stripped from the penis but is left attached and the urethra is cut. The excess skin is used to create the labia and the vagina..

Duration of the operation:

The surgery usually takes 6-8 hours to complete according to the extent of the procedure.

Post operative care:

After surgery patient should remain on bed for the first 24 hours.
The surgeon will perform the necessary care and do the dilation at least 2 weeks.
Patient must dilate to achieve the desired depth and width of the newly made vagina. If patient will not dilate properly, the result would be narrowing of the width and depth of the newly made vagina due to scar contractures.
After each dilation session with the doctor, wound dressing should be performed to speed up healing process.
Patient should avoid strenuous activities which can interfere with the circulation.
The vaginal packing, all drains and urinary catheter are usually removed on the fourth or fifth day following surgery.
Increase fluid intake at least 8-10 glasses per day to promote urination.
Wound dressing is done daily to prevent infection.
The surgeon will remove the stitches after 5-10 days.

Recovery period:

The patient should stay in Thailand at least 2-3 weeks to complete the whole process and allow proper healing and at least 1 week hospital stay for observation and management.
Patients can resume taking hormonal medications in 10-14 days after the surgery. Consult the doctor for adjustment of the dosage.
Patient can start vaginal dilatation 2 times a day for the first few of months about 1 hour each time with an adequate amount of lubricant. Vaginal stents typically range in size from about 1-1/8 to 1-1/2 inches or more in diameter (28 to 38 mm), and must be inserted to full depth (4 to 6 inches or more).

Possible risk and complications:

Patients must keep in mind surgeon’s instructions to prevent potential risk and complications following surgery. Possible complications include: Hematoma (bleeding), delayed wound healing, swelling, scarring and numbness or change in sensation, infection, hypertrophic scarring, partial or complete flap necrosis, intra-vaginal hair growth, recto-vaginal fistula.

Anesthesia use:
The procedure requires a general anesthesia.

Hospital admission:
You will be requested to sign in hospital consent form. Then you need five nights admission to hospital for recuperation.

Requirements to have sex change surgery?
According to “The Harry Benjamin International Gender Dysphoria Association’s Standard of Care for Gender Identity Disorders “a patient must qualify on the following criteria prior to the surgery:

Legal age of maturity in the patient’s nation. At least 18 years of age and will require permission from parents.
At least 12 months of continuous hormonal therapy for those without medical contraindications. Patient should present a Hormonal therapy certificate.
At least 12 months of continuous full time real life experience in a cross gender role.
Patient should present a Certificate of recommendation from a qualified psychiatrist/psychologist dealing with transsexuals for the diagnosis of gender dysphoria.
Physically fit for the surgery.

The goal of Sex reassignment surgery for Male to Female:

To create an erotic sensate area including clitoris, labia minora, labia majora and vaginal opening.
To achieve satisfactory vaginal depth.
To preserve orgasm during sexual intercourse.

Preparation before the surgery:

Criteria for approval are as follows:

Letter of recommendation from a psychiatrist.
Discontinued hormonal therapy at least 14 days prior to surgery. Hormones should be stopped before surgery to reduce the risk of thrombosis (blood clots).
Physical check up prior to SRS. Laboratory test should be taken. It includes the following: CBC, HIV, FBS, Creatine, Urinalysis, Alkaline phosphatase, SGOT LDH EKG and chest x- ray. You can have your private doctor perform the physical examination and send the results via email.
Patient must stop smoking 2-4 weeks prior to the surgery. Smoking interferes and delays wound healing process.
Patients must stop taking medications such as anticoagulants (aspirin), anabolic steroids and supplements (Vit. E) at least 4-6 weeks to prevent complicating medical factors prior to surgery.
Assessment of the medical history and physical examination will be performed during the consultation with the surgeon as well as explanation of the course of surgery, recovery period, risk and complications.
No intake of food or water 8 hours prior to surgery.

One month before the surgery:

Prepare and Plan: Put your schedule together for the day before, day of, and first few days following the surgery.

Preoperative tests: Make certain all test ordered (CBC, EKG, X-ray) by your surgeon MUST be completed at least 5 days before surgery and no more than 30 days before surgery. Test results should be received by Dr. Juta for clearance. If medical clearance is not yet received at the expected date of completion, surgery maybe delayed or cancelled.

Stop smoking: Smoking must be avoided for about 3-4 weeks prior to surgery, as nicotine interferes with circulation and will greatly affect healing process.

Stop taking medications: Avoid taking any medications such as hormones, anticoagulants, anabolic steroids and supplements at least 4-6 weeks to prevent complicating medical factors prior to surgery. Avoid taking aspirin, anti-inflammatory drugs and herbal supplements as they can increase bleeding.

Stop drinking alcohol: You will likely to be asked to stop drinking alcohol, a week before the surgery and throughout your recovery period.

Vital information: A preoperative visit or call is essential to review your health, your goals and any vital information including allergies and health considerations.

One week before the surgery:

Confirm your day of surgery: Review all orders and instructions and put your schedule together a few days before the surgery.

Continue to practice healthy habits: No smoking, no drinking alcohol and stop taking medications to ensure safety.

Relax: Get plenty of rest. Anxiety is common over the thought of surgery, so don’t hesitate to contact our office. We are here to support you and answer all of your concerns regarding surgery.

One day before surgery: Pack your personal belongings for the day of surgery.
Nothing by mouth. No candy, gum or mints. Anything more than a small amount of water as needed for brushing teeth or swallowing medication may result in the need to cancel surgery.

Dress appropriately: do not wear jewelry, hair clips, body piercing, cosmetics. (If there is something you cannot remove, let the admitting nurse know right away)

Wear comfortable, clean, loose- fitting clothing: Do not wear jeans or any tight fitting bottom; rather have a pair of loose, drawstring sweatpants to wear home. You may wear a robe. Wear slip on, flat shoes with a slip proof sole; no heels. Wear clean cotton socks as the operating room can feel cool.

During the surgery:

Appointment for consultation with the surgeon. The discussion will be regarding medical health history as well as the explanation of the course of surgery, recovery period, risk and complications. Don’t hesitate to ask questions for clarification and the technique to be used for SRS.

During surgery, your vital signs will be watched closely.

After your surgery, you will be taken to a recovery area where you will stay for about an hour.
A nurse will closely monitor you as you wake up from the anesthesia, checking your vital signs frequently.
If you feel pain, ask your nurse to give you pain medication.
Once you are awake and your vital signs are normal, you will be taken to your own hospital room and your family will be able to visit you there.
You will be discharged from the hospital as soon as your surgeon believes that you are ready and are not experiencing any complications from the surgery or anesthesia.
Genital electrolysis must be done prior to SRS to prevent hair in and around their new vagina.
Please remember that the effects of the anesthesia medications go away very slowly. Do not plan on doing anything important the day of or day after your surgery. You will feel drowsy for hours after your surgery. For these reasons, you MUST NOT drive a car, work with machinery, or sign any important papers for at least 24 hours.

It is important that you follow the written instructions given to you when you are discharged.
